Qualifications:
If patients have one or more of the following resulting from Diabetes Mellitus, they may be entitled to a pair of Therapeutic Shoes with Molded Inserts:

History of partial or complete amputation of the foot.

History of previous foot ulceration.

History of pre-ulcerative callus.

Peripheral neuropathy with evidence of callus formation.

Foot deformity
-
Poor Circulation
Medicare Guidelines:
Each Patient is entitled to the following per calendar year :
- One Pair of Depth Shoes
- Three Pairs of Custom Inserts
