Requirements:
The following is required in order for patients to be evaluated and fit for Therapeutic Shoes :

•  Patient Information Sheet
•  Doctor’s Order

Doctor’s Orders should outline the following per Medicare:

“Dispense one pair of depth shoes and
three pairs of  molded inserts for the
prevention of poor circulation and other
diabetic foot complications.”
